1/15/2023 EnchantaVaughn Approved Behavior: Client was observed being a good listener while QMHP reviewed his objectives and goals. Client was monitored and supported for exhibiting the correct manners and social skills. Client displayed appropriate body cues and voice volume in social setting. Client displays good social behaviors, laughing and playing safely with. As the client prepared for family session, he was seen helping his mother and displaying responsible behaviors. Client was engaged and focused, utilizing his free time to build healthy relationships with his family. Behavior: Client was observed being a team player and working well with others. Client was able to role play skills he learned in yesterday's psycho social and implement them in today’s psycho social. Client was acceptive of feedback. Client was doing an amazing job coping and staying calm during challenging tasks. Client affect was sunny, as characterized by his smile and happy facial expressions. Client shows excitement when completing his work without needing help. Client was praised for his hard work and commitment with his goals. Intervention: QMHP praised client for staying on task. QMHP complimented client for utilizing acquired skills, paying attention, and focusing on the importance of the assignment. QMHP supported client displaying positive social interaction with peers and authority. QMHP inquired about at home activities. QMHP will collaborate with mother regarding behaviors and performance when a counselor isn’t present. QMHP applauded client for being a leader. QMHP modeled language for client to use while in the home and in the community that will promote pro-social behaviors and effective communication. Intervention: QMHP reiterated to client the benefits of following the rules and of the privileges he can receive when he does so. QMHP assisted client with identifying privileges that he would like but does not receive due to his negative behaviors; in efforts to stimulate his thinking about cause and effect of his behavior choices. QMHP provided simple non-verbal praises as client was observed displaying positive behaviors. QMHP provided redirection when needed and processed with client on what he needs to do to achieve ISP objectives every day. QMHP applauded client for his commitment and efforts with doing his best and having a great turn around this week. Response: Client smiled and hugged QMHP for being there for him when he needed. Client displayed positive interaction with his peers and was supported with building healthy relationships in other settings. Client is more confident, as evidenced by assertive and proud tone. Client have been doing a better job controlling impulsive and hyperactive behaviors, due to keeping his hands to himself and complying to home rules. Client reduces sadness and finds more excitement in learning about different ways to control his reaction to certain situations and be more receptive and open in therapeutic setting. Response: Client was mindful of his family needs and made sure to thank them for giving him more opportunities than he had in any other place. Client verbalized he was grateful of receiving new things and promised to take care of it. Client utilized the resources provided and seem to be doing better with managing symptoms of ADHD. Client was ready to work and was more motivated to try and stay positive for the remainder of the day. Client displays appropriate behaviors at this time. Client demonstrates problem solving by staying calm and using his senses. Client acknowledges expectations and reviews goals and objectives with counselor.
